    New referral letter from your primary health care provider
    Prior operation reports or hospital discharge summaries
    Previous relevant X-rays, CT scans, MRI scans, Ultrasounds, bone scans and their reports
    Medicare card, and/or private health insurance details, workers compensation or 3rd party details
    Bring a family member, carer or trusted friend to assist you and ask questions
  • What to expect during your appointment
    A comprehensive history of your problem will be taken, please bring a summary of prior treatment and operation/s
    A focused clinical examination will be performed - please wear appropriate clothing that will allow for assessment of the problem
    Review of prior investigations and imaging to assist in diagnosis and guide treatment
    Options for further investigations and assessments will be discussed for additional information to assist diagnosis
    A referral will be provided to a trusted network of local physiotherapists and specialist colleagues to facilitate rehabilitation and further care
    Options for non-operative treatment and operative management will be discussed to assist in your recovery and rehabilitation
  • Partnership with primary health care providers, specialist colleagues, physiotherapists and allied health to maximise your function
    We will collaborate with your health care providers to maximise your recovery and healing
    Communication will be sent back to your primary health care provider and other treating team members to update treatment plan and share results
    A referral may also be made to a trusted network of specialist colleagues to assist and progress your recovery
    A referral will be made to a network of highly qualified physiotherapists and allied health providers to maximise your mobility and functional recovery
    A followup appointment will be scheduled to guide your recovery. Please obtain further imaging prior to next appointment as required.
